Social Services Referral and Navigation Worker

Job description
The Referral and Navigation Worker (RNW) is responsible for all general aspects of the position. The Referral and Navigation Worker will ensures that program goals are maintained and developed according to contract guidelines. The RNW will be responsible for intake, screening and referral to appropriate resources across community networks.
The RNW has some case management responsibilities.
