Registered Nurse - Full Time- Community Health Center- 5- 8's

Summary of Position:  The Registered Nurse (RN) works with all members of the Patient-Centered Medical Home (PCMH) Care Team by performing a variety of clinical and administrative patient-related duties as established in the roles and responsibilities defined below. The RN Actively participates in team huddles and care team meetings.  The RN provides professional nursing care, that is safe and effective, utilizing established policies and procedures.

The RN assesses, plans, implements and evaluates care in collaboration with physicians and practitioners. The RN provides leadership and supervision to all clinical staff.
