Transitions LifeCare is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Hospice Home team on the night shift. In this role, you will provide hands-on personal care and support to hospice patients while helping ensure their comfort, dignity, and quality of life. You will work closely with nurses and the interdisciplinary team in a supportive, mission-driven environment focused on exceptional end-of-life care. Full-Time | Nights (7:00 PM – 7:30 AM)
Schedule: 3 twelve-hour shifts per week | Every other weekend rotation (2 weekends every 6 weeks) | One winter holiday and one summer holiday annually
