Supported Living Community Inclusion Coordinator

Make a direct impact by supporting individuals with developmental disabilities in building meaningful community connections. Managing a flexible weekly caseload of 8–9 clients, you will provide personalized one-on-one support during outings, drive clients to local activities, and assist with personal care as needed. Duties include conducting client assessments, updating six-month care plans, and documenting billable activities following Developmental Disabilities Administration (DDA) guidelines.
