The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Regional and Specialist team includes Integrated Specialist Services, Mental Health and Wellbeing Services, Primary Healthcare Services, Birthing and Early Childhood Services (South), and Child and Youth Services. Our purpose is to connect, guide, and empower teams and communities through culturally safe, transparent, and responsive ways.
About the Role? As Health Wellbeing Worker – Pamela Mam Health Centre, your role is the familiar face, connector point, and navigator for which Mob can ask for help and guidance in their autonomous access and use of culturally responsive holistic primary health care. Through their close connection with clients, families, and community; working alongside their Pod and using all clinic roles and additional services to their license, the HWW will achieve continuity of care and improving the health status of Aboriginal and Torres Strait Islander people through: Welcoming, empowering, and connecting clients and families to (and back into) the service.
Providing health and social determinant information and education to clients and families which promotes autonomy and choice. Work alongside clients and families in their goal setting and prioritisation of health and social needs. Implementing plans, based on goal setting, in collaboration with the core multidisciplinary team.
About You? To perform this role, it is essential that the incumbent is an Aboriginal or Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 Current Criminal History Check. Current C Class Drivers Licence (QLD). Current Working with Children’s Check (Qld Blue Card).
Current CPR Certificate. Relevant Certificate III Qualification (such as Aboriginal and/ or Torres Strait Islander Primary Health Care, Certificate III in Community Services, Certificate III in Allied Health Assistance or equivalent) OR demonstrated experience working in an equivalent role. Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities What Can we Offer?
