The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

The Regional and Specialist team includes Integrated Specialist Services, Mental Health and Wellbeing Services, Primary Healthcare Services, Birthing and Early Childhood Services (South), and Child and Youth Services. About the Role? As Cardiac and Pulmonary Rehabilitation Nurse, your role is responsible for specialist cardiac and pulmonary rehabilitation nursing services for Aboriginal and Torres Strait Islander people with cardiac or pulmonary conditions. As a member of a multi-disciplinary team, you will ensure effective screening, service delivery and administration practices are delivered in accordance with the IUIH Model of Care.

The role will entail program delivery across a number of locations in SEQ, under limited supervision, as required. Your day-to-day duties will include: Delivering care within your scope of practice, observing universal precautions and clinical guidelines Liaising and coordinating care with tertiary providers (e.g. hospitals, specialists, agencies) to ensure safe, seamless follow-up and referrals.

Coordinating and delivering group education sessions that are culturally relevant, engaging, and tailored to client needs and strengths. Providing individual support and facilitating referrals to appropriate internal or external services in consultation with the client and care team. Delivering cardiopulmonary rehab programs in an autonomous but collaborative way, working closely within an integrated Rehab Team.

About You? Current C Class Drivers Licence (QLD) and own reliable transport. Current AHPRA Registration. Bachelor of Nursing Qualification (Registered or Enrolled Nurse). Current First Aid and CPR Certificate. Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities What Can we Offer?
