We are looking for a highly organized, people-savvy Scheduling Coordinator to join our growing home care team in Stratford, CT.

As the heartbeat of our daily operations, you will be the central point of contact coordinating schedules for our dedicated field staff, ensuring our patients receive seamless care, and keeping our daily workflows running perfectly.
What You’ll Do (Key Responsibilities) Master the Logistics: Coordinate, build, and optimize daily and weekly schedules for field nurses and clinical staff. Smart Matching: Assign patient visits strategically based on geographic location, clinician availability, and specific patient needs. Real-Time Problem Solving: Manage daily scheduling adjustments on the fly (handling call-outs, urgent new admissions, and route changes).
Clear Communication: Serve as the primary bridge between clinicians, patients, families, and internal office teams. Intake Support: Collaborate with the intake team to ensure swift, seamless coverage for all new patient referrals. Data Integrity: Keep our scheduling systems up-to-date, accurate, and compliant. What We’re Looking For (Qualifications) Career-Minded Professional: A reliable, proactive, and solutions-oriented individual looking for long-term professional growth and stability.
The Right Experience: Proven background in healthcare administration or coordination. (Experience specifically within home health care or utilizing Electronic Visit Verification (EVV) software is a massive plus!) Chaos Coordinator: Exceptional organizational skills and the ability to stay calm and focused in a fast-paced environment.
Communication Skills: Excellent phone etiquette and the confidence to communicate effectively with diverse populations. Tech-Savvy: Comfortable navigating digital scheduling platforms, electronic health records (EHR), and standard office software. Language Skills: Bilingual in English and Spanish is highly preferred (but not required).
