Job Summary

We’re looking for skilled and caring Registered Nurses to join our Connecticut home care team. This role offers the autonomy of field work, the support of an experienced clinical team, and the chance to build meaningful patient relationships. Part-time opportunity 3 days per week Responsibilities Provide skilled nursing visits and complete comprehensive assessments Create, implement, and update individualized care plans Deliver patient/caregiver teaching, medication management, and wound care Communicate regularly with physicians, therapists, and the care team Document all care accurately and on time Maintain professional boundaries and promote patient safety at all times Qualifications Active Connecticut RN license (required) Preferred: Home health experience, including OASIS Accepted: Hospital, med-surg, telemetry, LTC, SNF, or rehab Strong clinical assessment skills Valid driver’s license, reliable vehicle, and auto insurance Excellent communication and documentation skills What We Offer Competitive pay structure Supportive, accessible clinical leadership Orientation + ongoing training Career growth as we expand statewide The ability to make a real difference through 1:1 patient care Work Location: In person
