Job description
The Community Outreach Specialist provides support services to consumers residing in an independent housing program and in the community. The support services include coordinating client case management, planning and carrying out goals of individual's treatment plan/ISA, goal setting, training independent living skills, and supporting individuals' overall mental health stability and general wellbeing.
The Community Outreach Specialist will provide transportation of day-to-day appointments as needed. This support service assists individuals to reside independently in the community through skill development and education. This position allows for interfacing with a variety of individuals with differing needs and approaches.
