Job description
The Community Clinician provides assessment, treatment planning, and therapeutic services to adult clients in their homes or community settings, with a primary focus on older adults experiencing transportation or mobility barriers. This role helps ensure individuals have access to high-quality mental health and substance use services where they are most comfortable.
Working closely with the Council on Aging and an interdisciplinary clinical team, the Community Clinician enjoys the flexibility and independence of community-based work while benefiting from a collaborative team environment, supportive supervision, and dedicated office space for documentation and care coordination.
