Job description
MAIN FUNCTION: Medication Therapy Management. The MTM Pharmacist provides comprehensive medication management services to optimize medication therapy outcomes, improve patient safety, and enhance quality of care. The pharmacist works collaboratively with physicians, advanced practice providers, nurses, and other healthcare team members to identify and resolve medication-related problems, improve adherence, educate patients, and support evidence-based medication use.
EDUCATION
Pharm D degree in pharmacy EXPERIENCE: State of CT registration/licensure required.
