Job description
We are seeking dedicated and compassionate individuals to join the team at Polara Health, where our mission is to provide accessible, compassionate whole person care that strengthens our community. As an Adult Case Manager, you will work with the outpatient Adult Services team at our Windhaven Center office, providing support to help adult clients access services and stay in treatment. We offer a team approach to health and wellness; medical providers, therapists/counselors, case managers, and peer support team members work together to support the mental health and well-being of our clients.
