Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT)to join our growing Home Health team in Los Angeles County. This Per Diem Physical Therapist roleoffers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist**, you will work with senior care populations**, including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy servicesin home-based, assisted living, and community care settings, helping patients regain independence and improve their quality of life.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health**, you will provide skilled rehab services** tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independencefor senior and geriatric patients.

Key Responsibilities:

• Conduct in-home physical therapy evaluations and reassessments**
• Develop and implement personalized rehabilitation care plans**
• Improve strength, balance, mobility, and functional outcomes**
• Provide therapy in home health, assisted living, and community care settings**
• Educate patients and families on fall prevention and safe mobility techniques**
• Document visits using WellSky (Kinnser) EMR**
• Collaborate with interdisciplinary home health teams**

Qualifications

• Licensed Physical Therapist (PT) – California (Required)**
• CPR Certification (AHA approved)**
• Minimum 1 year of clinical experience** (Home Health preferred)
• Experience in senior care, geriatrics, rehab, or community-based care** preferred
• Strong clinical, communication, and patient education skills**
• Valid driver’s license, reliable transportation, and auto insurance**
