Your Role as a Home Health PTA (Physical Therapist Assistant)**

As a Physical Therapist Assistant in home health**, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility**.

Key Responsibilities:**

• Provide in-home physical therapy services** following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs**
• Support senior care patients** in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention**
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR**

Qualifications:**

• Active California Physical Therapist Assistant (PTA) license**
• Current CPR certification (AHA required)**
• Minimum 1 year of clinical experience** (home health experience preferred)
• Strong interest in home health, rehab, and senior care**
• Excellent communication and organizational skills
• Valid driver’s license, auto insurance, and reliable transportation
