LTC Lead Community Worker & Engagement Coordinator

The Long-Term Care (LTC) Lead Community Health Worker & Engagement Coordinator supports residents in a five-unit, 24/7 residential group home serving individuals with developmental disabilities across a range of ages and support needs. This role promotes wellness and independence through person-centered care, while developing and coordinating culturally relevant, meaningful daily activities that reflect community values and support resident engagement.
