CCBHC Evaluation & Treatment Planning Specialist

The CCBHC Evaluation & Treatment Planning Specialist position works within a multidisciplinary team using a client-centered and trauma-informed approach to conduct comprehensive diagnostic assessments and lead the development of individualized, person-centered treatment plans that integrate mental health, substance use, and physical health goals for clients across the lifespan.

This role provides high-quality, community-based behavioral health services to individuals with serious mental illness and substance use disorders, focusing on symptom reduction, functional improvement, and rehabilitation. Works with diverse staff and service population.
