Case Manager-Adult Psychiatry

The Case Manager is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered. The Case Manager will work closely with psychiatry providers and client's team to ensure they receive the necessary services and support to achieve optimal health and well-being.

The Case Manager will work in office and services can be provided to adult and child clients in office or virtually. This role involves assessing clients' needs, developing care plans, coordinating services, and advocating for clients' rights and needs.
