Community Care Coordinator (Outside Sales)

Hospice Community Liaison Albuquerque, New Mexico | Hybrid | Full-Time GREAT HOSPICE MARKETING STARTS WITH TRUST. We are seeking an experienced Hospice Community Liaison who understands the importance of trust, communication, follow-through, and genuine relationships.

Your role

will be to serve as a knowledgeable resource for physicians, facilities, case managers, patients, and families while developing referral partnerships throughout the Albuquerque community. You won't simply be making calls or dropping off marketing materials. You'll become a trusted point of contact for the people who rely on you to help connect patients with appropriate care.

WHAT YOU'LL DO

Build Relationships Develop lasting relationships with physicians, hospitals, SNFs, ALFs, assisted living communities, case managers, and other referral partners Maintain regular face-to-face contact with referral sources Identify new partnership opportunities Strengthen existing referral relationships Represent Billet Health at community and professional events Educate & Support Educate referral partners about hospice services Meet with patients, families, and caregivers to provide information about hospice and available resources Help families understand the role and benefits of hospice Work closely with the clinical team to support appropriate transitions of care Maintain strong communication throughout the referral process Own Your Market You will have autonomy over your territory and the relationships you develop.

You'll plan your outreach, manage your pipeline, identify opportunities, and work toward established business-development goals. WHY BILLET HEALTH? Billet Health provides more than hospice. WHAT WE'RE LOOKING FOR

At least 1 year of hospice marketing experience, or relevant healthcare sales experience with strong hospice knowledge Established referral relationships within the Albuquerque healthcare community Excellent interpersonal and communication skills Strong presentation and relationship-building abilities Knowledge of Medicare hospice guidelines and eligibility Understanding of HIPAA and healthcare compliance Self-motivated and organized Comfortable managing an independent territory Valid driver's license Reliable transportation and current auto insurance Preferred Experience SNF, ALF, hospital, or physician-office marketing Proven referral-generation results EMR/CRM experience HOW WE MEASURE SUCCESS Success in this position means building relationships that last.
