Case Manager-HSSP

Under the direction of the program manager provides comprehensive and intensive case management services to adults with severe and persistent mental illness. Case management services include linkage to community resources such as housing, medical, educational and vocational services. In addition, provide rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports clients in meeting their goals.

Adheres to all required documentation standards.
