ACT Housing Specialist

As a member of an Assertive Community Treatment (ACT) team, functions as the housing specialist with primary responsibility for the provision of housing support services in accordance with SAMHSA’s evidence-based practice of permanent supportive housing. Provides comprehensive assessment of housing, residential and activities of daily living skills and needs, and leads in service planning for adult daily living skills and housing support services.

Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.
