50/mile for distance covered between patient home appointments Unit: Home or Community Shifts available: Days Shift timings: 8 Hours; Days, 8a – 5p Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri with occasional back up for weekend and on-call. Contract offer flexible scheduling Position Requirements: Registered Nurse: 1 to 2 years

• Required Hospice Nursing experience: 1 to 2 years – Required EMR/EHR charting
• Required Must have valid Driver’s License and Personal Auto Insurance Education, License and Certification Requirements: ASN or an equivalent – Required BSN or an equivalent – Highly preferred RN License for State applying or Compact if accepted in the state
• Required BLS Certification from AHA or American Red Cross
• Required Job Description: Provide Nursing Care for Hospice Patients in private homes, nursing homes, group homes or assisted living facilities. Manage assigned caseload of patients assigned caseload of patients and lead the interdisciplinary team caring for those patients Provide assessments, care planning, pain management and symptom management and all other aspects of hospice nursing Administer Physician-prescribed medication Ensure timely and accurate submission of patient documentation and charting in adherence to client policies. Territories to cover: Grand Junction, CO and surrounding areas
