• Required Recent/Current Hospice LPN nursing experience in a home health setting: 1 year within the last 3 years – Required Home Care Home Base (HCHB) charting experience – Required Must have valid Driver’s License and Personal Auto Insurance Must be Covid Vaccinated or provide medical or religious exemption Education, License, and Certification Requirements: LPN License for the State of Ohio or Compact
• Required BLS Certification from AHA or American Red Cross
• Required Job Description: Provide and coordinate care for assigned Hospice patients under general supervision Develop and implement nursing care plans for patients as assigned. Empower patients and families to reach healthcare goals by educating them on disease management. Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians and IDT team of significant changes in patient status.
