Wellness Community Health Worker

The Wellness Community Health Worker supports patients managing chronic health conditions by connecting them with healthcare services and community resources. Working closely with Wellness leadership and care teams, the Community Health Worker provides care coordination, health education, and support for chronic disease management and prevention.

This role also serves as a liaison to the Wellness Center, helping facilitate group programs focused on self-management, physical activity, and nutrition to support culturally responsive health promotion and reduce health disparities.
