Case Manager - Residential Corrections

Position Summary

As a Case Manager in Corrections , you’ll play an important role in supporting individuals as they transition into and successfully navigate life in the community. You’ll work directly with clients to identify needs, build individualized service plans, connect them with resources, and help them develop the skills and stability needed for successful community reintegration.

What You’ll Do Build professional, supportive relationships with clients while maintaining appropriate boundaries and promoting accountability.
Conduct client interviews, assessments, and screenings to identify strengths, needs, risks, and service priorities. Manage an assigned caseload and maintain consistent, meaningful contact with clients to monitor progress and address emerging needs. Develop individualized service plans in collaboration with clients and regularly evaluate progress and make updates as needed.
Apply core correctional practices to help clients develop positive behaviors, strengthen decision-making skills, and successfully reintegrate into the community. Provide ongoing case management and support, which may include: Crisis intervention Advocacy and problem-solving Medication monitoring Transportation assistance Financial budgeting and life-skills support Housing assistance Referrals and connections to community resources Identify barriers to success and work with clients and community partners to develop practical solutions.
Maintain accurate, timely documentation of all services and activities in accordance with program, funder, regulatory, and agency requirements. Participate in team meetings and contribute recommendations regarding client progress, needs, and changes in status. Lead educational groups and/or facilitate established group curricula.
Transport clients when required as part of assigned duties. Maintain compliance with applicable laws, regulations, professional standards, and agency policies. Protect client confidentiality and consistently demonstrate ethical, culturally responsive, and respectful professional conduct. Perform other duties as assigned.
What You Bring We’re looking for someone who can balance compassion with accountability and stay focused on helping clients make meaningful progress. Successful candidates will demonstrate: Ability and willingness to learn and apply case management principles and core correctional practices. Strong organizational and time-management skills, with the ability to manage multiple priorities and meet deadlines.
Excellent verbal and written communication skills. Strong ethical judgment and professional boundaries. Ability to build rapport and work effectively with people from diverse backgrounds in a respectful, culturally sensitive manner. Strong problem-solving skills and the ability to remain calm and solution-focused in challenging situations.
Ability to work independently while also contributing effectively as part of a multidisciplinary team. Comfort using technology, including electronic service documentation and case management systems.
Education
& Experience Bachelor’s degree in Behavioral Science, Criminal Justice, or a related field , or equivalent work experience as permitted or required by applicable regulatory bodies. CDCA or other specialty certification may be required, depending on program or position requirements. Valid driver’s license and current automobile insurance required.

& Physical Demands This position involves a combination of office, community, and residential-based work and may require driving as part of assigned job duties. Physical requirements may include: Frequent standing, walking, and sitting. Frequent to constant reaching, handling, and talking. Infrequent balancing, stooping, kneeling, or crouching.
Rare climbing or crawling. Frequent use of near- and mid-range visual acuity and visual accommodation. Occasional use of far visual acuity, depth perception, and field of vision.
