Personal Care Specialist (CNA/STNA)

Join our hospice team in Wooster, Ohio , and provide compassionate, patient-centered care that promotes comfort, dignity, independence, and quality of life during one of the most important times in a patient's journey. We are seeking a dependable and caring Personal Care Specialist (CNA/STNA) to provide hands-on care to patients in their homes while working closely with nurses, caregivers, families, and the interdisciplinary hospice team. Position Details Location: Wooster, OH Job Type: Full-time Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m. Weekend & Holiday Rotation: Required Travel: Local travel throughout the Wooster service area, including Wayne, Summit, and Medina counties What You'll Do As a Personal Care Specialist, you will help patients remain comfortable, safe, and independent in the familiar surroundings of their own homes.
Your responsibilities
Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility Provide compassionate, respectful, and patient-centered care Promote patient comfort, safety, privacy, dignity, and independence Observe patients for changes in condition and promptly communicate concerns to the appropriate member of the care team Accurately document care and services provided using electronic documentation systems Communicate effectively with nurses, caregivers, family members, and other members of the hospice team Develop trusting and supportive relationships with patients and their loved ones Follow each patient's individualized plan of care Follow hospice policies, procedures, and applicable care standards Provide dependable care while traveling between patient homes throughout the service area Qualifications High school diploma or GED required Current Ohio STNA/CNA certification preferred At least one year of healthcare or direct patient-care experience preferred Basic computer skills and ability to use electronic documentation systems Valid driver's license and safe driving record Reliable transportation and required automobile insurance Ability to travel locally between patient homes Ability to work safely in varying weather and driving conditions Hospice certification (CHPNA) encouraged within two years of employment Why Choose a Career in Hospice?
