RN Care Manager Position Overview Ohio’s Hospice is seeking a compassionate and dedicated Registered Nurse (RN) Care Manager to provide high-quality hospice care to patients and families throughout Franklin County. This is a full-time, field-based nursing position requiring local travel to patient homes and care settings.
Schedule: 40 hours per week Monday–Friday, 8:00 AM–4:30 PM Weekend rotation approximately every 6 weeks Two holidays per year The RN Care Manager works closely with patients, families, caregivers, physicians, and members of the interdisciplinary hospice team. The ideal candidate is committed to compassionate end-of-life care and understands the importance of supporting both patients and their loved ones.
Responsibilities
The RN Care Manager provides skilled nursing care and coordinates services based on each patient's individual needs.
Key responsibilities include
Assessing patients and developing individualized plans of care. Providing nursing interventions and evaluating patient outcomes. Monitoring changes in patient condition and communicating appropriately with the care team. Educating patients and families about care, comfort, medications, and available hospice resources.
Collaborating with physicians and interdisciplinary team members to coordinate patient care. Maintaining accurate and timely clinical documentation. Following professional nursing standards, organizational policies, and applicable regulations. Providing compassionate support to patients and families throughout the hospice journey.
Traveling to patient locations throughout the assigned service area.
Qualifications
Graduate of an accredited nursing program. Current, unrestricted Registered Nurse (RN) license in the State of Ohio. Current CPR certification. Valid driver's license and safe driving record. Ability to maintain state-required minimum automobile insurance coverage. Ability to travel during daytime hours and, when necessary, in varying weather conditions.
Computer skills sufficient for clinical documentation and communication. Successful completion of required background screening and 10-panel drug screening. Preferred: Two years of acute care nursing experience. Hospice or palliative care experience. Certified Hospice and Palliative Nurse (CHPN) certification.
