Cary Medical Management (CMM) is a North Carolina-based medical services organization (MSO) managing a network of eight primary care clinics under a single tax ID across the greater Raleigh-Durham and Triad regions. CMM operates across seven value-based contracts spanning Medicare, Medicaid, and commercial payers — managing more than 60,000 attributed patients. Our results reflect what disciplined VBC execution looks like in practice: VBC income grew from 9.2% to 15.4% of encounter revenue, a 20% EBITDA increase, costs running 16% below the Medicare benchmark, and 15.7% below the Medicaid benchmark. We partner with ACOs, IPAs, and health plans, including Evolent, Privia Health, UNC Health Alliance, WakeMed Key Community Care, and Community Care Physician Network. Our technology stack — built on Smartlink Health Solutions and Infina Connect — enables seamless EHR integration, AWV and CCM program execution, risk adjustment, and care gap closure without disrupting existing clinical workflows.
