About Central Liverpool Primary Care Network

The Central Liverpool Primary Care Network (CLPCN) is a collaboration of six GP practices working together with local health, social care, and voluntary services to deliver coordinated, person-centred care to the people of Liverpool. Based at our main hub on London Road (L3 8JA), we combine the strengths of traditional general practice with the resources and expertise of wider integrated teams to improve health outcomes and reduce inequalities. Our focus is on providing proactive and preventative care, empowering patients to take control of their own health, and connecting them to the right support. Guided by our priority areas — Alcohol, Mental Health, and Supporting Children and Families — we are committed to building strong partnerships and ensuring the long-term sustainability of local primary care services to enhance wellbeing for all.
