About Heart City Health

Heart City Health is a non-profit, Federally Qualified Health Center (FQHC) in Elkhart, Ind., that provides affordable, high-quality, medical, dental, and behavioral healthcare. Heart City Health also features an on-site pharmacy and provides many other support services. They accept Medicaid, Medicare, and most private insurances. As an FQHC, they are also able to offer a sliding fee scale which is based on income and family size. As Level-3 accredited Patient-Centered Medical Home, Heart City Health knows the importance of having a doctor who knows your specific health history and understands your individual needs throughout every stage of your life.
