Journey Home for Empowered Living and Dying is a registered charity dedicated to changing the way our communities experience serious illness, dying, and grief. We believe that death is not a medical event alone it is a human, relational, and spiritual experience deserving of reverence, dignity, and personal choice. Serving the Greater Hamilton Area, Journey Home Support offers free end-of-life education and support, providing individuals & families with compassionate guidance long before death arrives. Through Peace of Mind Planning, we help people understand their options, make informed decisions, and articulate their wishes clearly, ensuring that end-of-life care reflects the values, culture, legacy, and personality of the life lived. Our support is non-medical and person-centred, guided by the belief that meaningful death should be rooted in love, community, and personal choice. We are MAiD-positive, offering emotional, spiritual, ritual, and planning support for those navigating Medical Assistance in Dying with dignity. Our doulas create space for honest conversations and help families approach MAiD with clarity, compassion, and respect. We help individuals and families honour the last chapter of life through ceremony, ritual, and celebration. From goodbye parties, legacy activities, and bedside vigils to sensory-rich rituals that include music, scent, touch, story, and nature, we help families create peaceful and meaningful memories at the end of life. These moments of beauty do not erase sorrow they dignify it, transforming fear into connection and grief into love. Our work extends beyond the bedside through community education for professionals, caregivers, volunteers, and anyone seeking to better understand death, dying, and empowered living.
