We work with you and your loved ones and the relevant social care team, as well as registered and social housing providers to identify a suitable property you can call home. This all helps build a support package catered to your individual needs and challenges. We support individuals in their own homes to develop independence and achieve their full potential. We have a number of dedicated purpose built services which offer individuals secure tenancy via housing association, separate to agreed care and support. Supporting people to move from hospital, residential care or family homes to live in their own accommodation whether this be on their own or friends.
