To align with our patients’ seven-day, on-site schedules, Mana Health Partners staffs full-time on-site nurse practitioners at your facility 7 days a week. Our nurse practitioners are dedicated to a single facility to form lasting relationships with patients, understand their unique needs, and implement personalized care plans. From routine check-ups to chronic disease management and timely intervention for acute issues, we provide skilled nursing facilities, assisted living, and post-acute facilities, with full-time onsite primary care.
