MeaeCare provides in-home clinical visits that help health plans improve risk accuracy and close quality gaps in a way that actually holds up under review. Our teams complete comprehensive assessments, screenings, and documentation during a single visit, then return that information back to the member’s primary care provider so it can be used. A lot of programs today still split outreach, care, and documentation across different vendors or workflows, which is where things start to fall apart. Data gets lost, diagnoses don’t make it back to the chart, and plans are left chasing gaps they thought were already addressed. We built MeaeCare to fix that. By connecting the visit, the data, and the documentation into one process, we make it easier for plans to capture what’s actually going on with the member, support stronger RAF and STARS performance, and avoid the rework that comes from incomplete or unusable records. The goal isn’t just to complete visits. It’s to make sure those visits translate into something measurable.
