Founded in 2006, Media Home Health has been a trusted award-winning provider of home health care services for nearly two decades. In 2016, we expanded our services to become an Assisted Living Waiver Program (ALWP) care coordination agency, further enhancing our commitment to supporting the health and well-being of individuals in need. With over 1,000 home health clients served and more than 3,500 clients assisted in applying for the Assisted Living Waiver Program (ALWP), our team is dedicated to providing compassionate, professional care across Southern California. We have established strong relationships with nursing homes and assisted living communities from Los Angeles to San Diego counties, ensuring seamless care transitions and comprehensive support for our clients.
