About My Care Alabama

My Care Alabama is part of the Alabama Medicaid program called Alabama Coordinated Health Network (ACHN). We operate in three regions under the name My Care Alabama Northwest, My Care Alabama Central, and My Care Alabama East. Our team provides care coordination services to Medicaid recipients within these three regions. Our care coordination team comprising of Social Workers and Nurses meet recipients face to face to provide services that help them improve their health.
