Onboard Outreach Inc. is a 501(c)(3) nonprofit organization dedicated to supporting individuals with special needs and their families across New Jersey. We provide person-centered planning, community-based supports, and resource navigation designed to promote independence, dignity, and meaningful inclusion. Our work focuses on helping individuals and families understand their options, access services, and build sustainable support systems. We assist with Supports Brokerage, Medicaid and SNAP renewals, Acumen enrollment support, housing assistance, and community-based service coordination. We believe support should be clear, accessible, and empowering. Whether navigating public benefits, identifying community resources, or building individualized plans, our approach is rooted in structure, advocacy, and compassionate guidance. Onboard Outreach serves families throughout North and South New Jersey and collaborates with providers, support coordinators, and community partners to ensure individuals receive the services they need to thrive.
