About Pathway - Homeless and Inclusion Health

Pathway is the UK’s leading Homeless and Inclusion Health charity, working with the NHS and other partners to create improved models of care for people experiencing homelessness and other excluded groups, including Gypsy, Roma, Traveller communities, people who do sex work, and vulnerable migrants. In the last three years, our Pathway Partnership Programme has helped 9 NHS Trusts in England to create in-reach Pathway teams of doctors, nurses, social care professionals and housing specialists who support over 3,500 patients experiencing homelessness every year. Many of the people that our teams support have complex combinations of physical illness, mental illness and substance misuse problems, and histories of trauma and abuse. Pathway’s model of multi-disciplinary teams in hospital settings uses the opportunity of an admission to enable people experiencing multiple exclusion to move onto a more positive life path. Pathway further works to improve the quality of healthcare that people experiencing homelessness and excluded groups receive by: • developing and sharing best models of care like our hospital teams; • sharing knowledge and increasing the specialist skills of the inclusion health workforce; • influencing policy; and • reducing stigma in healthcare against people experiencing exclusion. Pathway is also hosts the Faculty for Homeless and Inclusion Health, a national, multidisciplinary network for people involved in healthcare for excluded populations. The Faculty works to develop and improve practice in the field of inclusion health by setting standards, sharing knowledge, and supporting services in which generosity, kindness, and compassion combine with a passionate commitment to professional quality of care. It also organises the annual International Pathways from Homelessness Symposium, a sector-leading conference bringing together the best practice, policy, clinical and intervention work with vulnerable and excluded groups.
