PHE is a renewed primary healthcare concept with a team based approach aiming to provide quality preventive medicine with continuum of care. Our experience shows that this approach tackles a multitude of challenges faced by the patient in our current fragmented health system; and brings better compliance, fewer hospital admissions, better long term health outcomes, and significant reduction in the healthcare expenditure. Our uniqueness lies in the entire healthcare team (including physicians, specialists, physiotherapists, nutritionists, speech therapists, occupational therapists, cognitive therapists, care coordinators working in concert) being available to the customer at the clinic, at home, and for tele-consults including continuous remote monitoring. This approach enhances the patient’s access to their care provider and increases patient trust & satisfaction. Also, maintaining continuity of care throughout the patient’s health journey ensures better long term results in contrast to the current practice of episodic management. Managing most needs at home including home ventilation, post hospitalization rehabilitation, palliative care ensures comfort and convenience to the patient as well as the family, at the same time decreasing the load on our currently overburdened tertiary hospitals. Our vision is to develop a primary health care delivery model PHE: P (Preventive Personalized care clinic) . H (Home care) . E (Digital care); and create population based centers across urban India. As part of E-care, we are also working on a mobile application for early diagnosis and chronic monitoring of the common non-communicable diseases which form a major healthcare burden worldwide.
