Specialized outpatient palliative care, or SAPV for short, enables seriously ill people to remain well cared for in their own four walls, surrounded by their family and close to their friends in their final weeks, days and hours. - With its palliative care team, SAPV Care offers the following services: Care in familiar surroundings at home - Availability for patients and relatives 365 days a year, 24 hours a day - Multi-professional cooperation with GPs, specialists, clinics, care facilities and care services - Information and consultation with all those involved in the care process: patients, relatives, GPs, care services - Organization and coordination of all palliative care with forward planning: treatment of severely distressing symptoms such as pain, shortness of breath, nausea, vomiting - Creation of crisis plans, port care, wound care, use of pain pumps - Psychosocial support for patients and their relatives
