About Stichting Sociale Wijkteams Amersfoort
Stichting Sociale Wijkteams vervult een centrale rol in het sociaal domein van Amersfoort. Met ruim 180 wijkteammedewerkers leveren wij zorg en ondersteuning dicht bij de inwoners van de stad. De opdracht aan het wijkteam bestaat uit: vraagverheldering, ondersteuning en begeleiding én regie. Samen met een netwerk van organisaties versterken we zo de wijken. Daarbij blijven wij altijd kijken hoe de dienstverlening nog beter kan. Dit doen wij in het gesprek met de inwoners, tussen professionals onderling en met de externe partners.
