Co-created with the community using a person-centred care (PCC) approach to address the gaps - a web of complex issues that were identified among our clients, namely: Unequal gender roles in agricultural land access and use, limited health literacy, unhealthy food systems, and the rising burden of NCD risk factors among People Living with HIV A unique project based in Homa Bay that aims to empower women to access information on obesity, diabetes, and hypertension prevention and control through stakeholder and community engagement in health education and agri-nutrition project, which promotes physical activity, access to dietary diversity and household food and nutrition security. This project won the 3rd cycle WHO NCD Lab award as one of the best grassroots innovations tackling NCDs
