We are a general practice based in east Cheshire that is fully integrated with the local community and our social care partners. We formally merged four separate practices into one organisation in 2019. Now working together as the Middlewood Partnership, we are able to radically change the way we deliver care to our 34,000 patients in Bollington, Disley, and Poynton. Our integration with Team BDP -our Care Community partners- means we can manage patient care in the most effective, seamless way. Our clinical model, with a single digital point of access to GP services and wider community teams, allows us to embrace every request - allocating it to the right professional to deal with quickly and responsively. We are redefining general practice for the 21st century. We aim to make primary care more responsive, integrated, engaged and resilient; establishing a stable, sustainable organisation and improving care for patients in the process.
