Key Responsibilities

Ensure the delivery of high-quality care to residents and their families or representatives, in accordance with the organisation’s mission, vision, values, and strategic objectives. Promote resident safety and provide quality care while respecting each resident’s dignity, privacy, independence, and individual rights. Ensure that care services are delivered in an inclusive and respectful manner, without discrimination based on age, gender, sexual orientation, disability, family status, civil status, race, religion, or ethnic and cultural background.

Build positive relationships with residents to support their autonomy, informed choices, and evidence-based decision-making. Recognise each resident as an individual and prioritise their rights, personal needs, preferences, and comfort when delivering care. Adopt a management approach that promotes residents’ independence and maximises their ability to exercise personal choice and autonomy.

Ensure effective communication with residents in a manner that accommodates their individual needs and preferences. Identify, document, and communicate residents’ specific communication requirements to staff, ensuring these are incorporated into their individual care plans. Maintain ongoing, effective communication with residents’ family members, representatives, and visitors, as appropriate.

Promote continuity of care by ensuring relevant information regarding residents’ circumstances, medication, treatment, and ongoing support is communicated to healthcare professionals and subsequent care providers, where appropriate. Safeguard residents against all forms of abuse, including physical, sexual, psychological, financial, material, neglect, acts of omission, and discriminatory abuse, whether arising from deliberate actions, negligence, or lack of awareness.

Facilitate residents’ access to appropriate legal and financial advice whenever there are concerns regarding potential financial abuse by a third party. Arrange for a comprehensive assessment of each resident’s health, personal, and social care needs by an appropriate Health and Social Care Professional immediately before or upon admission to the nursing home.

Ensure an individualised care plan is developed within 48 hours of admission, based on the resident’s assessed needs, and made available in an accessible format to the resident and, where appropriate, their family members or representatives. Ensure that appropriate arrangements are made to meet a resident’s assessed needs if they decline to participate in the care-planning process, while taking their wishes, preferences, and aspirations into account as far as these can be established.

Ensure residents are informed about the personal information held about them, who can access it, including relevant professionals, and how they can exercise their right to access their personal information. Fulfil responsibilities in implementing and maintaining effective dementia and cognitive impairment care practices within the nursing home, ensuring residents receive appropriate, person-centred support.
