54/mile for distance covered between patient home appointments Unit: Home Health Hospice Shifts available: Days Shift timings: 8 Hours; Days, 8a – 5p Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No weekend, No call Position Requirements: Registered Nurse: 1 year

• Required Recent/Current Hospice Nursing experience: 1 year with the last 3 years – Required Home Care Home Base (HCHB) charting experience – Required Must have valid Driver’s License and Provide proof of personal Auto Insurance Must have reliable vehicle for patient visits Must be Covid Vaccinated or provide medical or religious exemption Education, License and Certification Requirements: ASN or an equivalent – Required BSN or an equivalent – Highly preferred RN License for State applying or Compact if accepted
• Required BLS Certification from AHA or American Red Cross
• Required Job Description: Provide and coordinate care for assigned hospice patients in comfort of their homes. Develop and implement nursing care plans including Start of Care and admissions as assigned. Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy. Empower patients and families to reach healthcare goals by educating them on disease management. Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians of significant changes in status. Document and complete charting for all patient visits within 24 hours. Provide bereavement support to patient’s families as needed. Territories to cover: Middlesex County, MA and surrounding areas
