Health Services Coordinator - LPN Preferred

Description Join Our Team and Make a Difference! The ARC of the Treasure Coast is seeking a compassionate and highly organized Health Services Coordinator to coordinate healthcare and medication services for individuals with intellectual and developmental disabilities living in our residential programs. This position helps ensure individuals receive safe, timely, and coordinated healthcare while supporting residential staff with medication and health-related needs.
Responsibilities
Coordinate medical, specialist, psychiatric, dental, diagnostic, hospital follow-up, and telehealth appointments. Track practitioner orders, prescriptions, pharmacy communication, medication availability, and MAR updates. Coordinate medication reconciliation following appointments, hospital visits, and medication changes.
Conduct monthly medication-system audits and follow up on identified concerns. Review medication storage, controlled medication counts, expiration dates, PRN documentation, authorizations, and consents. Assist with medication-error and near-miss reviews and help develop prevention plans. Train and support residential staff on medication procedures, documentation, and health-related processes.
Maintain accurate and confidential health records. Communicate with individuals, families or legal representatives, healthcare providers, pharmacies, hospitals, specialists, residential staff, and Waiver Support Coordinators. Ensure compliance with ARC policies and applicable APD and healthcare regulations. Travel between residential homes and healthcare providers as needed.
Qualifications
Current, unrestricted Florida LPN license preferred. At least 2 years of relevant experience in healthcare, residential services, developmental disabilities, behavioral health, long-term care, or medication management preferred. Experience working with individuals with intellectual and developmental disabilities preferred.
Knowledge of medications, MARs, practitioner orders, pharmacy communication, medical appointments, and health records. Strong communication, organization, and problem-solving skills. Ability to manage multiple priorities and maintain accurate documentation. Commitment to confidentiality and person-centered care.
Requirements
Valid Florida driver’s license, clear driving record, and reliable transportation. Proof of automobile insurance. Level II background screening. Ability to complete required APD and ARC training. CPR and First Aid certification. Bloodborne Pathogens and Medication Administration training. Physical Demands Ability to sit, stand, walk, bend, stoop, kneel, reach, push/pull, and work at a computer for extended periods.
Ability to assist with transferring individuals and pushing or pulling wheelchairs or other equipment as needed. Must be able to drive and lift up to 50 lbs.
