Social Worker – Skilled Nursing Facility Saugus, MA | Full-Time | Monday–Friday, 9 AM–5 PM Salary: $50,000–$130,000/year based on licensure, experience, and responsibilities Benefits Health, dental & vision insurance PTO, paid holidays & personal time Monday–Friday schedule Supportive regional team and stable leadership Opportunity for growth and expanded responsibility About the Role Saugus Rehab & Nursing Center is seeking an experienced Social Worker to join our skilled nursing team.

We’re looking for someone who understands the needs of long-term care residents and families and works well with an interdisciplinary team. LCSW or LICSW is strongly preferred , but we will consider experienced non-licensed candidates with strong SNF experience.

Responsibilities

Support residents and families throughout their stay Complete psychosocial assessments and required documentation Participate in care planning and interdisciplinary meetings Coordinate discharge planning and community resources Address resident and family concerns Collaborate with nursing, rehabilitation, admissions, and leadership The Social Worker reports to the Administrator and works closely with our regional team.

Requirements

Skilled nursing (SNF) or long-term care experience required Strong communication and organizational skills LCSW or LICSW preferred Compensation & Growth Compensation ranges from $50,000–$130,000 based on licensure, SNF experience, and responsibilities. Experienced licensed candidates may have the opportunity to support two facilities , with compensation reflecting the expanded role.
