This is a per diem role designed for flexibility—perfect for clinicians who want to pick up visits based on their availability without being tied to a full-time schedule.

Base (HCHB) Fast onboarding and consistent referral flow Responsibilities: Perform initial and ongoing patient evaluations Develop and implement individualized plans of care Provide therapy focused on activities of daily living (ADLs) and functional independence Complete OASIS assessments as required Educate patients and caregivers on treatment plans Maintain timely and accurate documentation Communicate patient status and changes with the care team Qualifications: Active Occupational Therapist license in the state of employment Minimum of one year clinical experience (home health preferred) Strong assessment and documentation skills Reliable transportation and valid driver’s license Ability to work independently in the field Compensation: $50 – $80 per hour equivalent, based on visit type and productivity as well as location Mileage reimbursement available Additional Information: PRN / Per Diem position No benefits offered
