NTSOC is looking for a compassionate LPN to provide high quality nursing care for our pediatric and adult clients to improve their quality of life at home.
Job Type: Full time and Part time.
Compensation
$24.00-$33.00 per hour dependent upon experience.

Implements the client’s physician ordered plan of care and nursing interventions while adhering to agency policies.
Perform assessments to monitor the health status of the client. Notify the physician and Field Nurse Case Manager of any changes in the client’s condition. Coordinate care with the Field Nurse Case Manager and other members of the client’s multidisciplinary care team. Administer medications and treatments as ordered.Document performed interventions and treatments.
Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming. Collaborate with the care team on implementation and updates to the client’s plan of care. Monitor client progress toward home health care goals. Educate the client and family regarding the plan of care and nursing care needs.

Minimum Education: Graduate of an Accredited Nursing Program required.Minimum Experience: 1-2 years home health care experience preferred and 1 year of nursing experience in a health care setting preferred. Strong communication skills. Strong computer skills.Licenses/Certifications: Colorado LPN license in good standing with the Department of Regulatory Affairs.
BLS required. Up to date car insurance and driver license will be required.
