Hospice Clinical Nurse Liaison (RN)

The Hospice Clinical Nurse Liaison builds and maintains responsive working relationships with acute care facilities and their clinical and care coordination teams. The position serves as a clinical point of contact for hospice referrals, evaluates referral information and patient needs, and coordinates timely evaluation and admission in accordance with hospice eligibility requirements, agency policies, and the Director of Clinical Operations.

The employee is responsible for evaluating and admitting referrals, as needed, for General Inpatient (GIP), outpatient/routine hospice, patients returning to facilities, and residential patients. The role also supports patients and families in understanding hospice services and coordinates transitions to the most appropriate setting.
