Job description
POSITION DESCRIPTION:#Assures continuity of care during the transition from the hospital to post-hospital care by assisting patients and their families with personal and environmental difficulties as they transition from CPH to home or other health care providers.

Bachelor Degree in#Social Work, MSW#preferred State of AK Social Worker Licensure or eligibility to obtain upon hire BLSP certification#
